I altered a child's dollar store costume (Dollar General) to fit an 18" doll using the following materials: 18" doll to take measurement from, mini candy container for the doll, a needle and thread to match the fabrics in the original dress-up.
In this case, it was by far less expensive to recycle an old costume into a new doll's costume than it would have been to collect the fabrics and notions needed from a sewing shop. So in the future, when matching dolls outfits to that of a child, purchasing two costumes at once, one for wearing, the second for cutting down to fit a doll. It just may be the most economical solution for some of you. Depending upon the expense of both, of course...
The original child's costume came with a headband that was too large for our doll. So, I removed the lady bug antenna and sewed it to a doll sized elastic headband made from the leftover scraps of fabric after cutting down material for a skirt and top.
I also gathered the tulle from the costume to make a ruffled collar around the neckline.
The little lady bug wings were simply removed from the original costume and tacked onto the backside of the blouse cut down for the doll. These were so small that they did not need to be changed in any way.
The doll wears small black Mary Jane shoes to finish her look.
